Dr Tan Yew Seng
Dr Tan has been a palliative care doctor for more than 25 years, working in various settings including general practice, hospice, community hospital, and acute private hospital settings.
Since 2024, he has returned to the community to start a new home palliative care team at Blossom Seeds Ltd.
He now hopes to enable and support lay people, whether they are family or volunteers, to care for and serve terminally ill patients at home.
Ms Lai Mee Horng
Lai Mee Horng is a Community Palliative Care Nurse with 42 years of nursing experience, including extensive knowledge in home-based palliative and renal supportive care.
She is dedicated to providing compassionate, holistic care that enables patients with life-limiting illnesses to remain comfortable and maintain dignity in their preferred place of care. Her areas of interest include symptom management, advance care planning, caregiver education, and end-of-life care.
Passionate about empowering patients and families through education and open communication, she is committed to improving quality of life and supporting individuals to live meaningfully throughout their end of life journey.
